Impact of ATA 2025 guidelines of thyroid cancer management on radiation safety procedures in clinical settings

Abstract
American Thyroid Association (ATA) guidelines for differentiated thyroid cancer have significantly changed between 2015 and 2025. The 2015 ATA guidelines recommended selective but relatively extensive use of radioactive Iodine (RAI) for low-risk and intermediate-risk patients as adjuvant treatment, residual ablation, and post-operative thyroglobulin monitoring. Accordingly, more radiation isolation facilities for patients, contamination control procedures, and radiation surveillance systems were required.
The revised ATA Guidelines and Clinical Updates for Thyroid Cancer Management, 2025, presented a more conservative and risk adaptive approach, which emphasizes personalized treatment, avoidance of unnecessary RAI administration in low-risk patients, and selection of intermediate-risk patients depending on treatment benefits. These changes are expected to reduce radiation exposure to patients, occupational exposure, and radioactive waste generation, with corresponding implications for radiation protection techniques and protocols.
This commentary analyzes how the radiation protection principles and methods are impacted by this shift in ATA guidelines. It particularly examines outpatient RAI administration, patient dose optimization, requirements for radiation isolation, occupational exposure, and radioactive waste management. Furthermore, how these updated guidelines align more closely with the core radiation protection principles of justification, optimization, and As Low As Reasonably Achievable (ALARA).
